
Seamlessly supplied at home in the network
The AAPV supplements the outpatient care provided by general practitioners and outpatient nursing services. It is provided by nurses with specialized training in palliative care.
The AAPV is aimed at people who suffer from a life-shortening, chronic, incurable disease with a stable disease process.
It is about identifying, preventing and alleviating discomfort and pain at an early stage.
We work closely with family doctors in the Hochtaunus district and fill in the gaps when your family doctor is unavailable: at night, on weekends and on holidays. This allows us to resolve emergencies locally and avoid unwanted hospital stays.
Our team consists of experienced palliative care specialists who are available to you 24 hours a day and regularly visit your home or nursing home.
The goal is for you to be able to remain in your familiar home environment and receive optimal care there.

Problem
You or a loved one is suffering from a progressive, incurable illness. The condition has worsened.
You are experiencing shortness of breath and pain. The symptoms are new — you haven't felt this bad in a long time. Your general practitioner is unavailable; it's the weekend.
In this case, you can call the AAPV (specialized outpatient palliative care).
Through the digital documentation system, the palliative care specialist can get an overview of your situation. Together with you, they will decide what needs to be done. If necessary, a doctor will be consulted.
In most cases, you can stay at home and do not need to go to the hospital.

Admission to the AAPV
To be accepted into the AAPV, we require your family doctor's consent in the form of a "Home Health Care" prescription. You can contact your family doctor about this or contact us directly.
The AAPV offer is free of charge for you.

Further information about the AAPV
The AAPV of Löwenzahn gGmbH was funded as a pilot project by the Hessian Ministry of Social Affairs and Integration for three years.
AOK Hessen is the first health insurance company to finance the AAPV for its insured members in the Hochtaunus district. For insured members of other health insurance companies, we are continuing to try to secure the offer through donations.
